My weapons disappear in combat

I can equip a weapon when i walk in towns and all, pull out the sword and so on but as soon i am in combat i cant.
I have to fight with my knuckles. Any idea whats causing this ?

Like in single player, your character has a combat and civilian outfit. You've equipped the gear in your civilian outfit.

Your inventory lets you choose which to equip on at the top of your screen.
